Continuum of Care (CoC) Examples

These two state examples show how the CoC structure can vary from state to state.

Wisconsin CoC Map

There are four CoCs in Wisconsin, of which three are in high-population regions: Milwaukee, Racine and Dane counties. The fourth is a "balance of state" CoC, meaning the remaining regions of the state are all part of a single CoC.

Wyoming CoC Map

By contrast, there is one, statewide CoC in Wyoming.
